Wittenberg self-insures its health care plan and is a member of the ELCA-related University & College Employees' Health
Benefit Trust. The university participates in a Preferred Provider Organization (PPO). For an employee who sees a PPO
provider, there is a $15 co-pay for office visits, and benefits are paid at 90 percent. For providers not a member of the
PPO, the benefits are paid at 70 percent.
A new employee has 30 days from the date of employment to elect coverage, and coverage is effective on the first of the
month following the date of employment. The university pays 80 percent of the cost of the monthly contribution, and the
employee is responsible for the remaining 20 percent. Plan types and current employee costs are as follows:

Wellness Benefit
The University's plan encourages preventive care. The plan pays 100 percent of the first $500 each
calendar year for routine physicals and related lab fees. No deductible will be applied to these services.
Prescription Drugs
Retail Pharmacy - Participants receive a 30 day supply and the cost is the lessor of 20% or $30.
Mail Order Pharmacy - To save money on the cost of prescription drugs, participants are strongly encouraged to use the Caremark mail service program for all maintenance medications. A 90-day supply costs $15 for generic medications or $25 for name brand medications. The first time that you use the service, you will need to submit a mail service order form with the prescription. Thereafter, you can order your refills on-line or by phone.

Dental Coverage
The Dental plan is not part of the PPO. The plan covers 100 percent of the cost for two
examinations/cleanings each calendar year, pays 80 percent for basic procedures, and 50 percent of major procedures up to the
maximum limit of $1,500 each calendar year for you and each of your dependents. Deductibles: $25 per person & $75 per family
Orthodontia benefits are available for dependents 19 years and younger. The plan pays 50 percent up to a maximum benefit of
$1,000.
